- Print and complete the Admission Health Record for Medical Students (PDF). Instructions for completion are listed on the form.
- Have your healthcare provider complete "PART II: Required Immunizations" in the Admission Health Record or submit copies of previous immunization records per the instructions.
- Review the required immunizations for medical students and associated deadlines which are also listed on the Admission Health Record.
Submit the Admission Health Record and proof of immunizations to:
